
A new study published in the journal of Supportive Care in Cancer found that olanzapine significantly improved the prevention of chemotherapy-induced nausea and vomiting (CINV) in cancer patients. The study examined the effectiveness of different doses of olanzapine, including 10 mg, 5 mg, and 2.5 mg, in preventing CINV.
Cancer patients often suffer from CINV, which can be debilitating. The normal 10 mg dose of olanzapine can prevent CINV, but it often causes incapacitating drowsiness. To address this issue, the study investigated whether lower doses of olanzapine could maintain antiemetic control while reducing toxicity.
Study Methodology
The study conducted a network meta-analysis of 54 randomized controlled studies that assessed the effectiveness of olanzapine in preventing CINV in patients undergoing chemotherapy. The analysis included 10,455 subjects and examined complete response, complete control, no nausea, and no vomiting as primary outcomes.
The study found that olanzapine-containing regimens significantly improved outcomes in all stages of CINV compared to placebo-controlled regimens. The 10 mg dose of olanzapine showed the numerically largest efficacy estimates, but direct comparisons with the 5 mg dose did not show statistically significant differences in effectiveness.
Key Findings
The study also found that olanzapine 5 mg was as effective as olanzapine 10 mg in patients receiving highly emetogenic chemotherapy. Additionally, olanzapine 5 mg significantly improved outcomes in patients receiving mildly emetogenic chemotherapy compared to placebo-controlled regimens.
Reducing the dose of olanzapine from 10 mg to 5 mg did not appear to increase the risk of sedation. They also suggested that olanzapine 2.5 mg may maintain effectiveness while reducing toxicity, although more evidence is needed to confirm this.
Overall, the study concluded that olanzapine-containing regimens significantly improved the prevention of CINV across all efficacy outcomes compared to placebo-controlled regimens. The findings of this study may help guide the treatment of CINV in cancer patients.
